Outpatient DXL

Diagnostic x-ray and lab (DXL) tests ordered or performed by a Doctor are payable at amount shown in the Benefits Table, per covered person, when hospital confinement is not required. Routine exams are not covered under this benefit. $300 maximum benefit per person per calendar year.
